OUR NEW PATIENT COORDINATOR WILL CONTACT YOU TO SCHEDULE YOUR APPOINTMENT and send you a link to the patient portal.

Step 4:

Create your account in the patient portal and follow the instructions for new patients.

Get ready to feel like yourself again.

» Initial Appointment:

Expect to spend about an hour reviewing your questionnaire and symptoms with our provider. Blood testing and Saliva testing will be prescribed at this initial appointment.

    » Initial Office Visit:

    » Saliva Testing:

    $310

     

    » Blood Testing:

    Done at your lab of choice. Your blood testing should be covered by your insurance, however it is important to check with your insurance company to determine coverage.

    » First Follow Up Visit:

    Usually scheduled 3-4 weeks after your Initial Appointment once you have completed your labs and testing. Test results will be reviewed and a treatment plan will be adopted at this appointment.

    » Second Follow Up Visit:

    Usually 3 months after your first follow up visit. This appointment is used to make any necessary adjustments to your treatment plan.

    » Third Follow Up Visit and beyond:

    Typically you will be seen every three months until you are stabilized. Once stabilized, you are seen every four to six months.
