Process and costs:

1. Complete the New Patient Questionnaire

2. A New Patient Coordinator will contact you to schedule

» Initial Appointment:

Expect to spend about an hour reviewing your questionnaire and symptoms with our provider. Blood testing and Saliva testing will be prescribed at this initial appointment.

    » Initial Office Visit:

    $425

     

    » Saliva Testing:

    $310

     

    » Blood Testing:

    Done at your lab of choice. This should be covered by your insurance.

    » First Follow Up Visit:

    Usually scheduled 3-4 weeks after your Initial Appointment once you have completed your labs and testing. Test results will be reviewed and a treatment plan will be adopted at this appointment.

    » Second Follow Up Visit:

    Usually 3 months after your first follow up visit. This appointment is used to make any necessary adjustments to your treatment plan.

    » Third Follow Up Visit and beyond:

    Typically you will be seen every three months until you are stabilized. Once stabilized, you are seen every four to six months.

    INSURANCE AND PAYMENT OPTIONS:

    • We do not participate with any insurance programs however your office visits, testing and prescriptions may be covered by your insurance company on the Out of Network portion of your insurance. For each visit, you will receive an itemized invoice with insurance claim codes and diagnosis codes that you can submit to your insurance company for possible reimbursement.  
    • Flexible Spending Accounts and Healthcare Spending Accounts are accepted. 
    • Interest free financing options are available through Care Credit at www.CareCredit.com. 
    • Our providers have opted out of Medicare and Tricare Programs. Patients are not permitted to submit claims to any Medicare and/or Tricare program for reimbursement. 

     

    RESCHEDULING AND CANCELLATION POLICIES :

    • A credit card is required to hold your appointment.  
    • We have a SEVEN DAY reschedule or cancellation policy. If you need to cancel or reschedule your appointment and do not give us at least SEVEN DAYS notice, you will be charged a $75 fee.  
    • If you do not show up for your appointment, you will be charged a $75 fee.
