At time of service we provide the patient with an itemized invoice that includes insurance claim codes for possible reimbursement by the insurance company directly to the patient. Our charges would fall on the out-of-network side of a PPO plan.
Medicare and Tricare patients: our providers have opted out of all Medicare and Tricare programs. Patients are not permitted to submit claims to any Medicare and/or Tricare program for reimbursement.
